To summarize, the company has applied to waive the requirement for a delisting EGM and exit offer under the Listing Manual (Rule 1307 and Rule 1309 respectively), reasons being given in the link above. SGX has no objection to both applications.

Can shareholder accept the offer partially on what they hold? Or must it be a all the share they owned or nothing acceptance? 

For example, if I hold 1000 shares, I exercise my sale right using Form 58 to sell 999 shares to the offeror. Then I continue to hold 1 share in the delisted company. Possible?

Anyone knows?

ya i received too. Is the witness really important? Still need to input their IC number. My parents won't understand what is this if I ask them to sign...

You can go to the share registrar office and ask one of the staff there to be your witness. Then, you sign on the spot and submit the form with the share certificate attached.

omg thank u so much ghchua!!. Also I didn't realise that the share cert needs to be submitted as well. Thank gdness i asked here.
Seems like they will send the proceeds via a cheque using ordinary mail. Just wondering what will happen if the cheque is lost in mail or somehow I didn't receive it?

Yes, they will send a cheque to you via ordinary mail within 7 business days after they have received your completed form. If the cheque is lost, you can always ask the share registrar to cancel the cheque and send you a new one.
